Referral Requests via Form Mail
Referrals -- you don't like 'em; we don't like 'em. Believe it or not, HMO's don't like 'em either, which is why they are making you and our staff do more and more of their clerical work to process them.
This is a new experiment to offer an alternative way of requesting referrals This is not the fastest way to get a referral -- it must be printed out, your chart still needs to be pulled from the file, and together they have to be routed to the referral desk and then to your doctor. But this may be less of a hassle for you than reading all this information over the phone to a receptionist or leaving a message and playing telephone tag. We hope it will promote accuracy, as well.

Because this system is new, we don't yet have a secure system. That means your form is readable as it goes around the internet. But you may feel comfortable using it for referrals you don't feel any need for confidentiality about. (Maybe you want the Department of Homeland Security to know you are taking good care of your glaucoma...)
