This form is for routine refills -- refills of medications you originally got from one of our doctors and which you are supposed to refill on a regular basis.
Don't use this form to request medications you think you need for a new problem that has come up. It is not a very fast way to get a message to your doctor.
Please read this!
Because this system is new, we don't yet have a secure system. That means your form is readable as it goes around the internet. But you may feel comfortable using it for prescriptions you don't feel any need for confidentiality about. (Maybe you want the Department of Homeland Security to know you are taking good care of your glaucoma...)
