Payment is due at the time of service, including all co-payments required by the insurance company you have chosen. If you do not have insurance that covers office visits, you will be required to pay for a minimum office fee at the time of registration and pay any balance afterward. We can only keep our fees reasonable by limiting credit to established patients with unusual hardships.
You must bring an up-to-date insurance card when you arrive for your appointment. If your card lists a primary care doctor (including "No PCP Chosen"), and our office is not on your card, please correct this before the day of your appointment. Otherwise you may need to either reschedule your appointment or else pay cash at the time of the visit and attempt to seek reimbursement from your insurer. Call us if you don't understand how to correct your coverage or if you have a medical problem too urgent to reschedule.
You are responsible for any charges your insurer does not cover. This includes any charges after you have left the job that provided the insurance if you do not pay for COBRA coverage. It also includes charges if you change your primary care doctor or terminate insurance coverage after our enrollment list is printed.
We accept many plans that all cover different things. In some cases we accept plans because we know our patients may have little choice, not because we agree with that insurer's policies. It is up to you to know what your plan covers or excludes.
We accept personal checks with a driver's license. This identification is required by the service that approves our personal checks, and our staff is not permitted to accept other identification. You are responsible for all fees due to bounced checks.
